The Cardiac Rehab Nurse reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.
The Cardiac Rehab Nurse coordinates the care of Phase I Cardiac Rehab patients at the hospital.
This position is Casual meaning the hours per week are not guaranteed.
The hours will vary based on staffing needs but would be between 6:30am-6pm Monday Wednesday and Friday. Tuesday and Thursday 7:30am-3pm. No Weekends or Holidays.
This position will cover Delnor and CDH (Wheaton) Cardiac/Pulmonary Rehab locations.
Responsibilities:
- Provides patient initial assessment & orientation.
- Develops rehabilitation plan that addresses the patients risk stratification risk factors & exercise/functional status.
- Develops patient rehabilitation discharge goals that promote transitioning the patient back into community health & exercise resources.
- Communicates rehabilitation plan & discharge to ordering physician(s).
- Provides & documents patient care per department policy & external regulatory requirements.
Qualifications :
Required:
- Bachelor of Science in Nursing
- BLS
- ACLS (within 6 months of hire)
- Current state of IL RN License in good standing
- Basic computer knowledge
- Ability to communicate effectively in English both verbally and in writing
- Excellent customer service skills
Preferred:
- One year of experience in pertinent clinical specialty
