Join our team as Registered Nurse MDS Coordinator in our Long Term Care in Wabasha MN and be the change you want to see in the world.

Whats Available:

• 0.8 FTE (32 hours/week)

• This will be a Day/Evening rotation and include regular hours as MDS Coordinator as well as RN LTC role.

• Schedule will include weekend commitment of every 4thweekend combination of on-call or hours worked.

What you will do:

• Provide LOVE MEDICINE to our patients and residents

• Responsibilities will include ensuring and assisting with compliance with regulatory requirements and maintaining the quality of resident assessments in accordance with the Minimum Data Set (MDS) guidelines.

• Playing a critical part in enhancing the quality of care for residents by ensuring accurate documentation and effective communication across interdisciplinary teams.

• The registered nurse (RN) focuses on the health and wellness of individuals families and populations in collaboration with other health care professionals by incorporating evidence-based practice in the management of acute illness chronic disease disability and end-of-life care.

• The RN utilizes the nursing process and aligns practice with organizational policies professional nursing standards and regulatory practice acts

• Partner with a team of providers medical professionals and managers who are here to support your success and that of our patients in their health journeys.

What you need:

• Minimum of an Associate degree in Nursing

• RN licensure to practice in the state of Minnesota upon hire

• Current Infant-Adult Basic Life Support for Health Care Providers

• Excellent communication and interpersonal skills to effectively interact with patients families and healthcare team members

• Ability to work independently as well as collaboratively within a team environment
