Job Summary
HSCSCCare Manager I (Hybrid)
V-Tech Solutions is seeking a CareManager Ito support children and young adults with special needs by providing comprehensive care coordination services. The role focuses on assessment care planning service coordination and collaboration with families healthcare providers and community resources to ensure quality member-centered care.
Key Responsibilities:
Manage assigned caseload
Conduct assessments and community outreach
Develop andmonitorindividualized care plans
Coordinate medical behavioral health and community services
Support discharge planning and transitions of care
Educate members and caregivers
Document care activities accurately
Qualifications:
Masters Degree in Social Work
Active DC LGSW license
Two or more years of caremanagement or community health experience preferred
CCM certification preferred
Spanish preferred
