Job Summary

We are working closely alongside with a local authority in Wrexham to assist with the appointment of a Personal Advisor on a 3.5-months contract highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 15.85- 20.75 per hour

Summary:

The Personal Advisor provides a statutory service ensuring the Council complies with the Leaving Care Act 2000 and meets key performance indicators. The primary focus is to meet the needs of young people (aged 15.9 months and older) who are leaving or have already left the care of the Authority. You will act as a dedicated point of contact coordinating services and providing advice to maximize life chances and ensure a successful transition to independent living.

Responsibilities:

  • Initiate professional relationships with care leavers and manage a caseload of young people aged post-18 addressing specific needs identified in their Pathway Plans.
  • Complete My Pathway Plan with young people by age 16 supporting its preparation and reviewing it regularly to meet statutory requirements.
  • Provide expert advice to young people carers and staff regarding benefit entitlements DWP applications and opportunities for education employment and training.
  • Advocate for the rights and interests of children and young persons while strictly adhering to safeguarding procedures for Child and Adult Protection.
  • Maintain high-quality electronic and hard-copy records attend Looked After Children Reviews and participate in regular team meetings to develop practice standards.

Qualifications:

  • Minimum of 5 Grade C GCSE passes (or equivalent) including Maths and English.
  • NVQ Level 3 in Health and Social Care or an equivalent qualification

Essentials:

  • Direct experience working with young people facing complex issues
  • Experience in prioritizing a diverse workload and adhering to statutory deadlines.
  • Good working knowledge of the Children Act 1989 the Children (Leaving Care) Act 2000 and the Social Services and Wellbeing Act 2014 (specifically Part 6).
  • A commitment to equality and diversity and an understanding of the importance of Welsh language and culture.

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires a Basic DBS
