Schedule:AM / PM /Evenings (after-school patients required)
We are seeking aFull-Time Speech Language Pathologist (SLP)to join our home health team. This role is ideal for clinicians who are passionate about servingboth pediatric and adult populations value flexibility and are committed to delivering high-quality patient-centered care.
Position Summary
The Speech Language Pathologist is responsible for theassessment diagnosis treatment and preventionof speech language cognitive-communication and swallowing disorders. The SLP provides services in accordance with physician orders and agency standards and supports both clients and caregivers to improve functional communication and quality of life.
What Youll Do
- Provideclinical oversight and guidanceto Home Health Speech Language Pathology Assistants (SLPAs)
- Deliver speech therapy services according tophysician orders
- Assess and evaluate patients therapeutic rehabilitative and functional status
- Develop and implement individualizedPlans of Care (POC)consistent with evaluations
- Participate in the development of each clients comprehensive treatment plan
- Identify develop or adapt activities that enhance daily functioning and quality of life
- Assess the home environment for safety concerns and recommend appropriate interventions
- Monitor and report patient progress or changes in condition to the Director of Nursing and physician
- Coordinate care with other healthcare providers and community settings (schools daycare centers workplaces adult day care residential facilities)
- Educate family members caregivers and other personnel on treatment goals and interventions
- Assist in identifying additional services needed by clients and families
- Complete and submit clinical documentation and progress summaries in a timely manner
- Participate in discharge planning as appropriate
- Ensure compliance with alllocal state and federal regulations
What Were Looking For
- Active Texas licenseas a Speech Language Pathologist (SLP)
- Ability to provide services toboth adult and pediatric patients
- Experience withswallowing assessments and techniques
- Strong clinical judgment communication and organizational skills
