Where: Dudley (This is a fully on-site position. Remote or hybrid working arrangements are not available for this role.)

Hours: 37.5 hours per week

Full Time Salary: 37184.90 - 43384.61dependent on experience (Pro rata for part time)

Contract Type: Permanent

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Hold a caseload of service users with co-occuring severe and enduring mental health with high level substance misuse
  • Undertake physical and mental health assessments for people who access our services whenindicatedby clinical need and ensureappropriate signposting/referral to primary care orappropriate secondaryhealth care services foridentifiedhealthissues
  • Provideeducation health promotion and screening for blood borne viruses (BBV).ProvideHepatitis B vaccinations whenindicated
  • Contribute to CGLs Naloxone Strategy by training staff and service users in the use of Naloxone. Ensuring Naloxone is distributed to service users at everyopportunity
  • Develop therapeutic alliances with people who have substance misuse problems and develop effective partnerships withthem
  • Ensure that procedures are constantly reviewed to promote best practice andevidencedbased practicein accordance withUK Guidelines on Clinical Management NICE guidance and CGL clinical protocols and procedures
  • Provideharm reduction advice to injecting service usersprovidinginformation and equipment and ensure all equipment is disposed of safely
  • Record and input client data and information in order that the serviceoperateswithin contractual administrative and financial requirements
