The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest Kettle & Stony Point Watford and Sarnia. Our interdisciplinary team provides primary health care health promotion and community development services.
The North Lambton Community Health Centre is currently seeking applicants for the following position:
Registered Social Worker
18 Month Contract - 37.5 Hours/Week
Commencing March 9 2026
The Registered Social Worker (RSW) provides counselling and other therapeutic functions to support the psychosocial needs of clients accessing services at the Community Health Centre. As a member of the multidisciplinary team the Social Worker focuses on improving health and social well-being using the social determinants of health framework when delivering services navigating systems and advocating for equitable access to and improvement of the multiple dimensions that impact health and well-being.
Qualifications:
- Current registration with the Ontario College of Social Workers and Social Service Workers and a member in good standing;
- Bachelor or Master of Social Work from an accredited university;
- Three to five years experience in a community setting or combination of community and hospital; experience in a primary care setting is an asset;
- Awareness of and understanding of the social determinants of health
- Ability to work with diverse communities with respect sensitivity creativity innovation cultural understanding non-judgement and facilitation skills;
- Ability to maintain complete and accurate records in accordance with the Centres policies;
- Strong computer skills including the use of MS Office products and Electronic Medical Records.
This position enjoys a supportive environment with flexible hours 22.5 hours per week Monday to Friday. The rate of pay for this full-time contract position is $36.62 - $43.49 per hour (on a six-step grid). The rate will be based on experience and education. The Healthcare of Ontario Pension Plan is also available. The incumbent will be required to work at our Sarnia site and other CHC locations as needed.
