Job Summary
We have immediate opportunities on our Transition Team for Licensed Practical Nurses / LPNs interested in helping infants children and adults receive complex care at home in Philadelphia PA! This team will support the transition of care from the hospital or from our HICU Health Services team to ongoing home care services. This role offers a unique opportunity to partner with CHOP and learn how to care for clients with rare and complex diseases. As part of the Transition Team you will will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Qualifications for Transition Licensed Practical Nurses / LPNs:
- Have a current license as a Licensed Practical Nurse
- NICU PICU ICU or home care trach/vent experience required
- Achieve a passing score on high-tech Adult and Pediatric simulations.
- Hold current CPR certification
- Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
- Availability to commit to an ongoing consistent schedule
- Ability to travel throughout Philadelphia County

LPNs at BAYADA earn a salary of $30-$45 per hour!
Rate is based on qualifications and experience for the Transition Team role.
Pay: $30-$45/hr.
