The Complex Care and Recovery Program is currently seekingtwo (2) part-time permanentRegistered Nurses for a Job Share arrangementto work on the Downtown Central Assertive Community Treatment team (ACTT).As membersof a community-based interprofessional team you will provide client-centered psychiatric and general nursing care and ensure secure and safe adherence to CNO standards of nursing practice. You will work in partnership with clients their formal and informal supports and the team to support clients successful integration into their communities. You will function as the primary worker for a number of clients while being available to the teams other registered clients and contributing in a discipline specific role. As part of this continuing care team you will follow clients should they require hospitalization and plan and coordinate services with the inpatient care teams. You will be expected to perform assessments develop implement and evaluate interprofessional service plans for clients who have schizophrenia and concurrent disorders. You will liaise with other services in the Complex Mental Illness Program and the community to coordinate care and you will refer and link clients to appropriate services and resources. You will be able to work with clients from diverse populations; contribute to program development; provide education support and supervision to students and participate in the research activities of the program. Your skills include the ability to make behavioural observations mental status assessments monitoring clients response to pharmaceutical and care interventions documentation and ongoing communication with other team members. You will have sound knowledge of emergency psychiatry crisis intervention addiction issues and severe and persistent mental illness. You will support and contribute to a healthy workplace that embraces diversity encourages teamwork and complies with all applicable regulatory and legislative requirements. This position involves shift rotation (days and evenings) and weekend work This position is located at the Queen Street site.
The successful candidatesmust be a member in good standing with the College of Nurses of Ontario. A current B.C.L.S. (CPR) certificate is will have two years of nursing experience. Preferred experience in psychiatric/mental health nursing. Preference will be given to candidates whopossess orare currently pursuing CNA certification in Psychiatric/Mental Health will possess excellent communication and interpersonal skills to enable you to develop the Nurse- Client relationship and work within a collaborative and diverse inter-professional leadership decision-making organizational andproblem-solvingskills arerequired. Preference will be given to those with community nursing experience and asolid understanding of Recovery principles and their application to client service. Knowledge of emergency psychiatry and crisis intervention principles and theory arerequired. The ability to work with clientsofdiverse cultural and social backgrounds is necessary. Computer literacy isrequired. Bilingualism (French/English) orproficiencyin a second language would be an asset.
