POSITION SUMMARY:
Under the supervision of the director of specialty pharmacy and clinical development and according to department policies procedures and guidelines the Specialty Clinic Pharmacist is responsible for the enrollment and ongoing prescription adherence monitoring for all patients referred by pharmacy liaisons for Specialty Care Management responsible for comprehensive medication reconciliation with electronic medical record of each patient during prescription verification. The Specialty Clinic Pharmacist serves as the direct point of contact to provide pharmacy clinical support for all enrolled specialty clinics when needed.

BS in pharmacy or PharmD from an accredited college of pharmacy required.

Compensation Range:
$104000.00- $151000.00This range offers an estimate based on the minimum job qualifications. However our approach to determining base pay is comprehensive and a broad range of factors is considered when making an offer.
