A Qualified Professional (10A NCAC 27G .0104) must have one of the following combinations of education and experience:
Bachelor degree (non Human Services) with 4 years full time experience with the population served required
Associate or Bachelor degree (Human Services) with 2 years full time experience with the population served required
Master degree (Human Services) with 1 year full time experience with the population served required
Other Required Qualifications/Skills:
Strong capacity to work independently and collaboratively demonstrating initiative and accountability.
Understanding of IDD populations and service definitions regulatory requirements.
Strong communication skills that allow for collaboration with individuals families staff colleagues supervisors and external stakeholders.
Strong technology skills and willingness to learn electronic health records system for documentation and review of information.
Conduct initial and ongoing assessments monitor individual progress and evaluate the effectiveness of personcentered plans (PCPs)
Facilitate the development implementation and modification of PCPs in collaboration with individuals team members natural supports and community partners
Ensure all PCPs and required documentation are completed accurately creatively and on time including daily monthly quarterly and annual EHR entries in compliance with agency state and federal standards
Promote and model personcentered and positive support strategies and integrate these approaches into program operations and staff training
Support individuals participation in community and shopping activities and ensure spending aligns with approved budgets
Ensure service requests meet eligibility admission and discharge criteria and that supports are clinically appropriate and fiscally sustainable
Initiate monitor and maintain clinical authorizations and ensure compliance with audit requirements documentation standards billing processes and financial management (petty cash individual funds expenditures and budgets)
Provide leadership supervision and guidance to staff including PRN and float staff while modeling consistent expectations
Conduct regular onsite supervision across all shifts including unannounced visits to monitor service quality staff performance and adherence to PCPs
Complete clinical supervision plans coaching counseling progressive discipline and performance evaluations as required
Assist with recruiting hiring onboarding and training in collaboration with administrative personnel
Maintain effective communication across shifts departments families and external partners to ensure seamless service delivery
Ensure staff schedules and daily operations meet individual needs and arrange appropriate program supervision when offsite including working shifts on the floor (e.g. sleepover shifts) as needed
Maintain a safe environment and respond promptly to incidents and crises in accordance with agency procedures; participate in the oncall rotation as scheduled
Perform driving and travel duties as required
Complete other related responsibilities as assigned by the Supervisor
