Job Summary
Hospice of Marion a part of Empath Health is seeking a Manager Access & Care Transition to join our Manager Access & Care Transition is an integral component of the leadership access team within the Business Development and Admissions Department with responsibility for ensuring quality and productivity standards related to patient access are met and are aligned with all internal policies and goals. For hospital referred patients this position is directly responsible for compliance with governing state and federal regulations and standards of care guidelines of outside accreditation agencies. This position has daily oversight for workflows and other staff members including nurses and business development team members to ensure timeliness and quality care. They lead efforts within the community related to education and enrollment for all care programs offered by Empath Health including Hospice Care Palliative Care Dementia Services etc. The Manager Access & Care Transition is required to develop and foster partnerships with multiple referral sources including community hospitals external healthcare partners patients families and other referral sources.

What Youll Need
Education and/or Experience: A.S. Degree required - Bachelors degree (B.A.) from four-year college or university and Masters degree preferred in business or healthcare related field.
Three years Hospice experience with at least three years of management experience preferred.
At least five years experience as an RN preferred.
Licensure: Florida RN
Continuing education: Must acquire continuing education and training related to regulatory and professional practices. Demonstrates advancement in knowledge and expertise.
Certifications: Valid CPR certification from the American Red Cross or American Heart Association; Current certifications as applicable to their professional discipline.
Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance a copy of which will be placed in the employees HR file. This position requires travel between multiple locations.
Must successfully complete critical thinking skills assessment.
