The Police Liaison reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.
The BHS/Police Coordinator is a full-time grant-funded position that reports to the Manager of Behavioral Health Services. Under the direction of the Manager this individual will work closely with all social medical and other community partners to link police involved citizens to appropriate levels of care. As requested or appropriate and agreed upon by the citizen this person will assess evaluate and formulate treatment plans for the continued care of the citizen. This position will entail research collection of data reviewing of police reports assisting officers with individuals exhibiting signs of mental illness and/or substance abuse community meetings department meetings and performing other functions related to provision of mental health/substance use services within the community.
RESPONSIBILITIES
- Assesses diagnoses and develops a treatment plan that addresses the patients mental health and addiction needs in collaboration with the patient/family.
- Responds to incidents with police when individuals are expressing signs of a mental health crisis.
- Develops an ongoing treatment plan with the patient/family for mental health and addiction.
- Provides individual/family/group therapy and teaches/reinforces psychoeducation and coping skills for patients and families.
- Assesses collaborates and coordinates patients discharge planning needs and readiness for discharge with other members of the healthcare team.
- Identifies and refers patients to appropriate community resources for follow-up care.
- Serves as liaison to contributing public and social service partners.
- Coordinates necessary reporting prepares reports and other pertinent documentation as requested.
- Reviews police reports to determine if follow up intervention is required and referrals to community resources and/or social services need to be made.
- Coordinates attends and leads multidisciplinary and community teams as directed.
Qualifications :
Required:
- Bachelors degree in Psychology Social Work or other Behavioral Health related field
- Valid Drivers License
Preferred:
- Masters degree in Psychology Social Work or other Behavioral Health related field
- LCSW LCPC or LMFT
