What Youll Do

As a Skilled Home Health LPN youll provide high-quality nursing care to patients in their homes under the direction of an RN or physician. Youll play a key role in helping patients stay safe comfortable and out of the hospital.

• Deliver skilled nursing care according to the plan of care and physician orders

• Support patients and caregivers with education and self-care techniques

• Observe assess and communicate changes in patient condition to the RN Case Manager

• Document care accurately and timely in the EMR

• Promote patient safety and help prevent avoidable hospitalizations

• Build trusting relationships with patients families and care team members

What Were Looking For

• Graduate of an accredited nursing program

• Active LPN license in the state

• At least 1 year of clinical experience (home health or hospice experience preferred)

• Comfortable working independently in the field

• Valid drivers license reliable transportation and ability to travel within Elkhart & LaGrange Counties

• Ability to meet physical demands of the role

• Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
