An opportunity has arisen for a clinical pharmacist to join South Uttlesford PCN to provide high quality patient focussed pharmacy services to the patients of the PCN.This position is offered on a full-time basis and based in/out of a variety of South Uttlesford practices/locations according to need.
Main duties will include;
- Patient facing long-term condition clinics to review the ongoing need for each medication
- Patient facing clinical medication reviews
- Patient facing care home medication reviews
- Patient facing and telephone medicines support
- Unplanned hospital admissions review of medications and reconciling of medications
- Repeat prescribing
- Identification of high risk patients

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
- As a minimum you will hold a degree in Pharmacy (Masters or equivalent)
- Be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society
- Ideally you will be a non-medical independent prescriber but the PCN will actively support you to achieve this if not already an independent prescriber
- You will be required to lead by example setting and maintaining high standards of professionalism within a team environment
- You should be IT literate and confident in using practice systems
We formed our primary care network to work together to enable us to operate more efficiently and ensure that a greater proportion of our resource is focused on clinical care and improved access
Our PCN covers 4 practices across West EssexPeacock Surgery Angel Lane Surgery John Tasker House supporting 54000 patients.
