Program Overview:

The Health and Wellness Cluster is a $10M division which provides a wide array of personalized behavioral health services to individuals coping with the challenges brought on by emotional problems and mental illness. As a New York State certified Article 31 mental health facility the clinic in this division delivers outpatient mental health services crisis services HIV family mental health services vocational rehabilitation parent education and case management. Services are provided by culturally competent staff who can communicate in Chinese and Spanish as well as English. These programs are designed to help individuals attain their full potential in an environment that values respect dignity caring and integrity.

Job Summary:

As a Fee-for-Service social worker you will provide Individual Family and Group clinical interventions to clients in our Outpatient and PROS program. Most of our clients reside in the Lower East Side of Manhattan range in ages from 9- 90years old and come from diverse economic and ethnic backgrounds. You will also conduct Intake interviews manage crisis experienced by your clients and participate in In-service training supervision and Compliance activities. This clinic is licensed by the Office of Mental Health and you will have to follow documentation guidelines in our Electronic Health Records.

Qualifications:

  • LMSW/LCSW/LMHC required
  • Minimum two (2) years clinical experience with individuals and families
  • Bilingual - English/Spanish or English/Chinese (Cantonese/Mandarin) preferred
  • Computer proficiency
  • Knowledge of AWARDS a plus
  • Excellent verbal and written communication skills
  • Organized and able to maintain charting up to date and according to state regulations

Responsibilities:

  • Provide individual group and family services
  • Provide case management as needed
  • Provide crisis intervention
  • Maintain all paperwork according to State regulations
  • Collaborative documentation with clients in session (i.e. patient participation in treatment plan)
  • Participating in Mandatory staff meetings
  • Work flexible hours as needed by clinical demand
  • Must agree to at least one late shift a week (1pm-8pm)
  • Other related duties as determined by Supervisor

Essential Physical Job Functions:

  • Ability to walk on sidewalks up and down stairs and throughout clinic consistently throughout each day
  • Ability to carry folders a tablet a notebook and cell phone (up to 20 pounds) for several hours per day
