Job Location: New York NY 10017
Work Type: Onsite
Contract Duration: 3 months (with possible extension)
Shift: Night Shift 11:00 PM to 7:00 AM

Pay Rate: $26/hour

Position Summary:

The Virtual Care Coordinator supports virtual patient care activities and promotes patient-focused care across a large healthcare system. This role assists telehealth remote patient monitoring and virtual hospital programs by ensuring quality safety efficiency and compliance with clinical and operational standards. The coordinator collaborates closely with clinical teams leadership digital technology partners engineering and telehealth vendors to optimize workflows improve patient outcomes and expand virtual care services.

The Virtual Care Coordinator shares accountability for clinical quality and safety outcomes and serves as a liaison between clinical and technical teams while providing real-time support and continuous patient observation.

Key Responsibilities:

  • Perform quality assurance peer reviews and program evaluations; recommend improvements.

  • Meet regularly with senior leadership to review quality metrics workflows and program expansion initiatives.

  • Lead and collaborate on case reviews implement solutions and monitor outcomes.

  • Triage and assign cases for virtual monitoring and observation.

  • Develop implement and support telehealth policies and workflows.

  • Maintain operational databases tracking performance metrics response times and quality outcomes.

  • Identify trends and participate in corrective action planning.

  • Train new staff on telehealth platforms workflows and documentation requirements.

  • Serve as liaison between physicians clinical staff technology partners and vendors.

  • Monitor telehealth consult orders scheduled consults and follow-up visits in the EMR.

  • Ensure telehealth sessions start on time and run smoothly.

  • Provide high-touch technical support to physicians and clinical staff.

  • Escalate patient safety concerns equipment issues and workflow challenges as appropriate.

  • Submit utilization and assignment reports each shift.

  • Maintain accurate documentation of patient behavior interventions and safety events.

  • Participate in meetings team briefs and continuous improvement initiatives.

  • Maintain punctuality attendance and a safe clean and functional work environment.

Clinical and Patient Safety Duties:

  • Provide continuous remote observation and surveillance of assigned patients.

  • Monitor early warning signs such as vital signs Foley catheter days and central line days.

  • Identify changes in patient condition or behavior and immediately escalate to the RN or physician.

  • Suggest discharge from remote monitoring when clinically appropriate.

  • Ensure patient privacy and HIPAA compliance at all times.

  • Assist with interpretation services using approved methods.

  • Ability to navigate multiple technology platforms simultaneously.

  • Strong problem-solving prioritization and decision-making skills.

  • Excellent written and verbal English communication skills.

  • High attention to patient safety quality and compliance.

  • Flexibility and ability to function effectively in high-acuity environments.

Experience Requirements:

  • Minimum of 3 years of direct patient care experience.

  • Experience with telehealth or remote patient monitoring.

  • Experience with policy development impacting telehealth programs.

  • Experience with quality assurance outcomes tracking and data collection.

  • Experience with workflow development and protocol drafting for telehealth programs.

  • EMR/Epic experience.

  • Experience identifying patient safety risks and changes in clinical status.

  • Experience collaborating with interdisciplinary teams.

  • Understanding of regulatory legislative and organizational policies affecting telehealth.

  • Experience communicating telehealth concepts to staff and patients.

  • Ability to consider language culture and disability in virtual care delivery.

Education:

  • Associate or Bachelors degree in a health-related field (preferred).
