Job Summary and Responsibilities
As our Social Worker you will be a compassionate advocate providing vital support and guidance to individuals and families facing complex challenges.
Every day you will conduct psychosocial assessments develop care plans provide counseling and crisis intervention and connect clients to resources. Youll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.
To be successful in this role you will possess exceptional interpersonal skills empathy strong knowledge of community resources and proven crisis management abilities fostering positive change and client empowerment.
- Conducts psychosocial assessments on referred Hospice patients and develops a plan of treatment.
- Provides short term therapy and crisis/grief counseling to selected Hospice patients and their significant other.
- Responsible for coordinating and implementing discharge plan from Hospice Health Services for specific patients as appropriate.
- Provides consultation services to other disciplines evaluating and interpreting psychosocial information to other professionals.
- Completes documentation as required by Medicare other third party payors and The Joint Commission.
Shift: Monday - Friday Days
Student loan repayment program as well as relocation assistance offered!
Job Requirements
- Masters from a school of social work accredited by the Council of Social Work Education.
- Oregon LCSW License within 24 months
- Oregon CSWA working towards an LCSW with one year experience in a health care setting will also be considered.
