Pagosa Springs Medical Centers Clinic has an opening for a Part-Time Integrated Behavioral Health Care Provider (IBHCP). The part-time work in PSMCs Specialty Clinic is twenty-four (24) hours per week. Must be licensed in Colorado as a Licensed Clinical Social Worker Licensed Professional Counselor or Licensed Marriage and Family Therapist. Compensation range is $31.96 to $49.41 per hour depending on experience.

PSMCs part-timepositions include the following benefits:

  • Some health insurance benefits are available to part-time employees.
  • PSMC and Provider each contribute 6% of earnings to a 401a Retirement subject to I.R.S. contribution and compensation limits and five-year vesting.
  • Voluntary participation in 457 plan.
  • PSMC will provide professional liability insurance coverage.
  • CME allowance of $3000annually (gross and subject to taxes) for part-time.
  • PSMCs Rural Health Clinic is a Colorado Health Service Corps site and eligible providers can apply for the CHSC loan repayment program. PSMC is a Public Service Loan Forgiveness eligible employer.

This position provides integrated behavioral health care to Rural Health Clinic patients. This type of care is delivered with a warm hand-off during the clinic day or is a scheduled visit. This position also provides typical social work supportive to patient needs. The IBHCP uses specialized education discretion and judgement to perform the following: 1. Assess the patients physical psychosocial cultural and spiritual needs utilizing evidence-based screening tools (PHQ9 GAD7 CSSRS etc.); 2. Assist in the coordination of appropriate services resources and referrals; 3. Advocate and provide support to patients and families having difficulty coping effectively with changing medical conditions and other social determinants that may affect their health and well-being; 4. Diagnose and provide short-term evidence-based behavioral health therapy in the outpatient Rural Health Clinic; 5. Refer to a higher level of care when required for diagnosis and medication management; 6. Facilitate remote therapeutic sessions with offsite providers (Psychiatrists Psychiatric NP etc.) via telehealth when needed and act as a liaison between the specialist and the primary care provider. Duties may be changed as clinic needs are modified.

Qualifications
  • Graduate of Accredited Masters Program corresponding to licensure.
  • LCSW LPC or LMFT license in Colorado required.
  • Beneficial to have two (2) years social work experience in a hospital or similar Clinic setting; referral to community resources; discharge planning; and/or case management or other related experience.
  • Knowledge of community resources hospital operations and excellent communication/presentation skills.
  • Ability to assess DSM 5 diagnose and provide individual family and group therapy.
  • Ability to perform suicide risk assessments and interventions to include out of area placements.
  • Prefer MAT training and implementation; substance abuse experience
  • BLS
  • Prefer experience with third party payment systems i.e. Medicare/Medicaid.
  • Basic computer knowledge.
