Pathways Hospice & Palliative Care is seeking a Full-Time Hospice Licensed Practical Nurse (LPN) who provides skilled nursing care to patients and families dealing with terminal illness in the home or facility setting.
Job Responsibilities
The Full-Time Hospice Licensed Practical Nurse (LPN) is responsible for providing skilled professional nursing care to assigned terminally ill patients in their place of residence as prescribed by the physician and in compliance with the standards of the Missouri Nurse Practice Act and the mission vision and values of Pathways Hospice & Palliative Care.
Qualifications
- Current LPN license in Missouri required
- Minimum two years of nursing experience required with recent Hospice experience or LTC/SNF experience preferred. Experience with issues related to death/dying and bereavement preferred
- Complies with accepted professional standards and practice
- Understands Hospice philosophy; principles related to death and dying and pain management
- Possesses and maintains current CPR certification
- Complies with Pathways Hospice & Palliative CareHealth Status requirements
- Has good communication skills and the ability to provide good written documentation in a timely manner
- Instructs and explains plan of care to patient and family/caregivers teaching as needed. Instructs and encourages patient efforts toward self-sufficiency and optimal comfort care. Uses additional services when appropriate
- Performs procedures and treatments with proper techniques and complies with standard nursing policies and procedures. Uses equipment properly to administer prescribed treatments
- Responsible for coordinating monthly supervisory visit with Registered Nurse
- Participates in the on-call rotation
- Practices cost containment with time and supplies
- Consults with physician patient and interdisciplinary team regarding coordination of treatment goal development and response to treatment. Identifies need for other disciplines
- Attends and participates in interdisciplinary team meetings as assigned
- Documents accurately in patients electronic medical record in accordance with Pathways Hospice & Palliative Care policies
- Participates in Performance Improvement activities
- Participates in in-services workshops and seminars. Assumes responsibility for continuing education and maintaining current knowledge
- Other duties as assigned
