In this role youll manage a full patient caseload perform comprehensive assessments develop and modify treatment plans implement evidence-based therapeutic interventions and document progress for continuity of care. Youll collaborate with physicians nurses and other therapists in a multidisciplinary team participate in care conferences and contribute to discharge planning and home exercise programs that empower patients to maintain gains after leave.
