What Youll Do as a Home Health RN

  • Deliver high-quality patient-centered care in the home setting

  • Maintain primary responsibility for assigned caseloads including assessment care planning implementation and evaluation

  • Conduct initial and ongoing nursing assessments addressing physical psychosocial and environmental needs

  • Make regular patient visits and re-evaluate care needs throughout the episode

  • Communicate significant findings and changes in patient condition to physicians supervisors and care team members

  • Update and revise plans of care as patient needs evolve

  • Ensure all documentation meets clinical regulatory and HIPAA standards

  • Collaborate closely with interdisciplinary team members to support optimal patient outcomes

What Were Looking For

  • Current unrestricted RN license (state of practice)

  • 1 year of clinical nursing experience

  • Willingness to travel within the branch coverage area (approximately 50%)

  • Valid drivers license dependable vehicle and current auto insurance

  • Associate or Bachelors Degree in Nursing (preferred)

  • Home Health or Hospice experience (preferred)

  • Experience with HomeCare HomeBase (preferred)

This position reports to the Clinical Manager or Clinical Supervisor.
