Home Health PRN Licensed Practical Nurse (LPN)

How Youll Make an Impact

Working under the direction of an RN or physician youll provide skilled home health services by:

• Delivering care based on individualized plans and physician orders

• Performing skilled nursing tasks aligned with LPN scope of practice

• Observing assessing and reporting changes in patient condition

• Educating patients and caregivers on medications treatments and self-care

• Promoting patient safety and identifying potential risks in the home

• Documenting visits thoroughly and accurately in the clinical system

• Partnering with RN Case Managers for care coordination and guidance

• Supporting interventions aimed at reducing unnecessary hospital visits

What You Bring to the Role

• Graduation from an accredited LPN nursing program

• Active unrestricted LPN license

• At least one year of hands-on nursing experience

• Home Health or Hospice background preferred not required

• Willingness to travel throughout the service area (approximately 50%)

• Reliable transportation valid drivers license and current auto insurance

• Ability to meet the physical demands of home-based nursing
