Rehab Resources is seeking a passionate energetic and skilled Pediatric Physical Therapist to join our team in a PRN B-3 and outpatient pediatric role providing services in the home and community throughout the Fond du Lac area. This is a consistent PRN opportunity (12 days per week) with after-school hours ideal for therapists looking for flexibility while still making a meaningful impact with children and families.

Provide B-3 and outpatient pediatric PT services in the home and community
Evaluate motor development mobility balance coordination and strength
Create individualized play-based treatment plans
Support children in meeting developmental milestones
Collaborate with caregivers service coordinators and interdisciplinary teams
Participate in B-3 team meetings as appropriate
Educate families to promote carryover in daily routines
Maintain clear timely documentation of goals and progress
