LOCATION:Woodrow Wilson Keeble Memorial Health Care CenterIndian Health Service100 Lake Traverse DriveSisseton SD 57262CONTRACT TERM:Period of Performance:Begins on date of awardoBase Period: 6 monthsoUp to Nine (9) optional 6-month extensionsSCHEDULE & HOURS:Work Week: Monday FridayDaily Shift: Typically 8 hours between 8:00 AM 8:00 PMoUsual shift: 11:00 AM 8:00 PMWeekend Hours: 4-hour shifts 1:00 PM 5:00 PMHoliday Coverage: May be required; alternative weekday off may be scheduledExpected Weekly Hours: 4050 hoursPROVIDER REQUIREMENTS:Experience:oMinimum 2 years recent (within past 36 months) Family Medicine/General Medicine experienceEducation & Certification:oBoard Certified Board Eligible or partial completion of an accredited residencyoCurrent BLS certificationComputer Skills:oProficient with Electronic Health RecordsLanguage:oMust speak read and write fluent EnglishHealth & Immunizations:oNo physical limitations preventing clinical workoProof of immunizations required: Hep B MMR Varicella Tdap Flu and PPD within past yearCLINICAL DUTIES INCLUDE: primary care for all ages; average of 16 patients/ appropriate referrals and follow through with recommended in the Patient-Centered Medical Home (PCMH) patients on conditions treatments and and document relevant patient history and culturally competent care to Native American compliance with IHS CMS and facility in assigned committees (QA Pharmacy & Therapeutics etc.) in ongoing training and staff in peer reviews per facility bylawsSUBMIT QUALIFIED CANDIDATES ASAP DEADLINE IS 8/14
