Purpose: The Care Navigator will assist in follow up of appropriate care when patients transition from care levels as well as ensure patients are receiving quality care through maintenance of quality metric reports.
Job Duties:
- Maintain active HealtheLink access.
- Responsible for following the patients movement through the duration of their time in the Emergency department or Hospital Admission
- Will be utilizing and working with HEALTHeLink program to monitor alerts on patients progress and documentations of occurrence during visits
- Assist with the completion of Transition of Care Management (TCM) visits for those patients that have been hospitalized.
- Completes log of all Admission Discharge and Transfer (ADTs) that demonstrates movement through systems as well as follow up and discharge reports in patient charts.
- Maintains quality service by establishing and enforcing organization standards through management of quality compliance reports from insurance companies ACO and SIPA.
- Maintains patient care database by entering new information as it becomes available; verifying findings and reports; backing up data.
- Assists all patients through the healthcare system by acting as a patient advocate and navigator.
- Provide transportation services on an as needed basis only after all other resources have been exhausted (i.e. CARTS medical transport etc.).
- Assist the Care Coordinators as needed.
- Participates in Patient Centered Medical Home team meetings and quality improvement initiatives.
- Contributes to team effort by accomplishing related results as needed.
- Willing to be diverse and mentor peers in all job duties.
- Performs other duties as assigned
Education/Skills/Qualifications:
- Associate degree in Public Health Nursing or Behavioral Sciences preferred or equivalent experience in the human services field.
- Self-disciplined energetic passionate innovative
- One year ambulatory or public health experience is preferred
- Excellent interpersonal and communication skills required
- Leadership and organizational skills essential
- Bilingual especially in Spanish is preferred
- Computer literate preferred
- Demonstrate sound judgment decision-making and problem-solving skills
- Knowledge of medical office practices procedures and equipment
- Knowledge of problem-oriented record keeping
- Correct English usage spelling and grammar
- Confidence to communicate and outreach to other community healthcare organizations and personnel
- Knowledge of community resources
