The Acute Care Program is seeking a full-time permanent Nurse Practitioner who will be working within the scope of practice of a Registered Nurse (Extended Class) RN(EC) in the Virtual Urgent Care Clinic. The Virtual Urgent Care Clinic provides timely specialized mental health care through a virtual emergency department model. When not focusing on the Virtual Urgent Care Clinic the successful candidate will function as part of the interprofessional team in the Bridging Service. The Bridging Service is a drop-in low-barrier rapid access service that provides short-term care for clients who have recently been discharged from an acute care inpatient unit or diverted/referred from the Emergency Department.

Centrally located within the Acute Care Program the service supports continuity of care through a range of clinical and psychosocial interventions. Services include medication management housing and financial support and psychosocial support. Reporting to the Manager of the Bridging Service the Nurse Practitioner works collaboratively within the interprofessional team and liaises with teams across other services. The successful candidate will provide comprehensive care to clients including: conducting health assessments; providing follow-up and treatment; offering expert consultation to clients and members of the interprofessional team; planning implementing and evaluating direct care; providing clinical leadership related to the delivery of care for patients and their families; and working autonomously and accountably in communicating diagnoses prescribing medications and ordering diagnostic tests. The Nurse Practitioner will lead and manage crisis responses including suicide risk assessments demonstrating knowledge of the Mental Health Act. The successful candidate will practice in accordance with the College of Nurses of Ontario practice standards the Canadian Nurses Association Advanced Practice Nursing Framework and all applicable legislative requirements. You will support a healthy workplace that embraces diversity encourages teamwork and complies with all regulatory and legislative requirements. Day and evening hours may be required. This position is located at 1051 Queen Street West.

You must be registered with the College of Nurses of Ontario as a Registered Nurse in the Extended Class RN(EC) with a specialty certificate in either Adult or Primary Care and possess a current BCLS. Preference will be given to candidates who possess or are currently pursuing a Masters in Nursing or a related graduate degree. CNA certification as a Psychiatric Mental Health Nurse is an asset.

You must have a minimum of two years experience or demonstrated expertise working with clients with mental health and/or addiction issues. Demonstrated ability to work independently and as part of an interprofessional team is required including the ability to establish and maintain effective therapeutic relationships with clients with complex needs.

Excellent communication and interpersonal skills are required along with demonstrated knowledge of mental health and addictions and a strong understanding of the clinical standards related to Nurse Practitioner scope of practice. Experience working with clients from diverse ethno-racial and cultural backgrounds is required as is the ability to collaborate with internal and external stakeholders to support excellence in client care education and research.

Strong organizational decision-making and problem-solving skills are required. Computer skills including word processing electronic communication and workload measurement systems are essential. Ability to work flexible hours is required. Bilingualism (French/English) or proficiency in a second language is considered an asset.

Please Note: This full-time permanent position is a 1.0commitment which may be subject to change as per the Collective Agreement. Additional hours may be available based on operational needs. This position is part of the ONA Bargaining Unit.
