This role is for a Home Health Physical Therapist Assistant (PTA) responsible for providing physical therapy services to patients in their homes within Salem OR and surrounding areas. The PTA will work 8-hour days Monday through Friday and is expected to manage a caseload of patients ensuring productivity standards are met. The position requires the PTA to have their own transportation and be willing to travel to patient homes within a specified radius.

Responsibilities:
- Provide physical therapy treatments to patients in their homes according to established plans of care.
- Monitor and document patient progress adjusting treatment plans as necessary.
- Educate patients and their families on therapeutic exercises and activities.
- Maintain accurate and timely documentation of patient visits using the electronic medical record system.
- Communicate effectively with the supervising Physical Therapist and other members of the healthcare team.
- Manage a caseload of patients ensuring productivity standards are met (30 productivity points standard).
- Travel to patient homes within a service area potentially covering a radius of up to 60 miles.
- Provide physical therapy treatments to patients in their homes according to established plans of care.

Required Skills & Certifications:
- Active Physical Therapist Assistant license.
- Basic Life Support (BLS) certification.
- Minimum of 1 year of skilled home health experience.
- Valid drivers license and reliable transportation.

Preferred Skills & Certifications:
- Experience with EPIC Remote Client for Home Health and Rover (phone app).

Special Considerations:
- Traveler is expected to have their own transportation.
- The facility can call off up to one shift per pay period (every two weeks).
- Primary series COVID vaccination in addition to one booster required.
