Position Summary
Discharge planning is the process of transitioning a patient from one level of care to the next. Ideally discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers. Discharge planner reports off to the Social Work team and staff.
Essential Functions:
Communicates effectively and routinely with patients families medical staff nursing staff other caregivers when arranging follow-up care
Keeps track of all Medicare forms and consent forms
Assists with discharge planning and processes.
Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of the discharge plan
Participates in case reviews to evaluate case management and progress.
Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan
Interviews and assesses patients and/or patients family caregivers and/or legal representatives
Determines prioritizes provides and/or arranges for needed internal and external services/interventions
Assisting with group activities
Required Skills:
Requires critical thinking skills decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Education/ Certification
Bachelors Degree preferred or 3 years of experience; Masters preferred
Experience in Healthcare strongly preferred
