Job Summary:
Location: Methodist Fremont HealthResponsibilities:
Care Coordination & Patient Advocacy Resource Navigation (55%)
Communicate benefits and limitations of available discharge services (private and community based) and placement options available to patients family and care team.
Address social determinants of health by connecting patient with community benefit organizations referring to internal resources.
Educate and influence patients to seek/accept available resources by building trusting relationships.
Communicate and arrange services for patients with limited financial resources.
Coordinate/communicate all preliminary and final discharge arrangements including patient transportation.
Collaborate referral to pharmacy for options on medication cost/frequency to assist in discharge planning.
Provide referral as appropriate to Adult Protective Services (APS) Child Protective Services (CPS).
Provide Medicare/Medicaid information to patient/family.
Facilitate referrals to post-acute agencies as directed by patient preference insurance/coverage limitations and agency abilities.
Assist patient and/or family with advance directives and living wills.
Assessment (30%)
Conduct assessments to include identification of prehospital level of functioning and current patient/family utilization of community resources.
Assess psychosocial needs impact of illness or injury on social support system level of functioning and financial situation/resources.
Cultural Responsiveness & Change Support (15%)
Develop team cultural competency by educating on cultural norms/beliefs of community members.
Support a culture that maximizes the acceptance of change.
Establish trust and rapport with patients in culturally and linguistically appropriate ways.
Promote positive relations when interacting with community partners visitors customers and co-workers.
Build strong relationships with commonly referred community benefit organizations.
Act as a trusted member of the community (i.e. participate in community events community coalitions or health fairs).
Other duties as assigned.

Job Description:
Education:
Bachelors degree in healthcare or human services related field.
Experience:
Previous experience in healthcare or human services related field.
License/Certification:
Current American Heart Association or American Red Cross Basic Life Support (BLS) required within 3 months of hire.
License in healthcare or human services related field preferred:
Certified Social Worker (CSW) or
Licensed Mental Health Practitioner (LMHP) or
Certified Case Manager (CCM) through Commission for Case Manager Certification
Skills/Knowledge and Abilities:
Ability to work with a diverse range of individuals and teams
Strong negotiation persuasion and problem-solving skills
Strong organizational and time management skills including ability to multitask and prioritize tasks
Excellent communication and interpersonal skills
Ability to build foster and maintain strong relationships with patients families interdisciplinary teams and community agencies
Knowledge and understanding of post-acute services community benefit organizations APS/CPS processes and payer/coverage limitations
Knowledge of social work or care management principles including social determinants of health
Skill in evaluating support systems financial limitations and community resource utilization
Ability to navigate Medicare/Medicaid guidelines and explain benefits to patients and families
Ability to support team readiness for change and promote flexible thinking
Ability to represent the organization in community events or coalitions
Proficiency with Electronic Medical Record (EMR) and Microsoft Office Suite (Word Excel PowerPoint etc.)
