Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members are essential.

Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
Preferred Skills & Certifications:
- EPIC experience
Special Considerations:
- Mixed pediatric/adult Emergency Department environment
Scheduling:
- Weekends required rotating weekends
- Shifts start at 1200 or 1900 based on unit needs
- No on-call requirement
- No block schedules
- Expected ratio 1:5
