HOPE HEALTH HEALING
As a Discharge Planner - Residentialyou will be responsible for ensuring that appropriate discharge plans are in place prior to residential discharge and appropriate clinical services are coordinated for the member to support the successful transition in the community setting and reduce the risk of readmission.
In addition you will educate clients and support staff on housing options treatment options and community resources available to clients when leaving residential treatment. (housing group counseling individual counseling PCP behavioral health medication management 12 step meetings supported education employment services peer support certification psychosocial rehab etc.).

QualificationsQualifications:
- High School Diploma/GED and three years of relevant experience in a behavioral health setting; OR
- An Associates degree and two years of relevant experience in a behavioral health setting; OR
- A bachelors degree in the behavioral health field; OR
- A non-behavioral health field bachelors degree and one year of relevant experience in a behavioral health setting.
- Must pass DCS Central Registry Search with no disqualifying events.
- Must be at least 21 years of age.
- Background check.
- Minimum typing speed of 25 wpm.
- Must have a valid Arizona Level 1 Fingerprint Fingerprint Clearance card or be able to apply for one within 7 working days of starting employment.
- Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
