We have an exciting opportunity for a Clinical Aged Care Assessor to join our team in North West Melbourne VIC this community-based role you will work closely with clients in their homes utilising your clinical expertise to conduct assessments to support individuals in navigating their aged care journey.

YOUR NEW ROLE:

The Single Aged Care Assessment System (SAS) has been established by the Department of Health to simplify and improve access to aged care services. SAS brings together assessments across the aged care continuum including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments presenting a unique opportunity to up skill in the aged care sector.

In this role you will work alongside experienced and supportive assessors to deliver streamlined high-quality assessment services that prioritise the client experience and support timely appropriate decision-making.

You will:

- Work independently within your allocated region conducting holistic client-centred assessments with a strong focus on wellness and reablement

- Apply your assessment expertise to understand each persons functional ability strengths risks and support needs

- Capture comprehensive client information and work collaboratively to develop clear support plans and meaningful individualised goals

- Support older Australians to remain living independently by identifying appropriate supports including assistive technology community services and social supports

- Use the Integrated Assessment Tool within My Aged Care to accurately document assessments and inform outcomes.

This role is responsible for completing care needs assessments for people aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander people) who require assistance with everyday activities. Assessments consider the full range of restorative physical medical psychological cultural and social factors that influence a persons care needs and quality of life.

YOU WILL BE RESPONSIBLE FOR:

Undertake client assessments and review and develop support plans to assist clients in achieving their goals and living independently in their own homes and community.

Encourage and support client choice and involvement in decision making when developing detailed care plans to effectively meet individual needs.

Commit to partnering with clients to facilitate effective engagement and participation.

Manage an assessment caseload.

Meet individual and team KPIs and targets in line with funding requirements.

Retain appropriate registration or equivalent relevant to the profession.

Participate in working group sessions to brainstorm ideas and contribute to solutions that lead to system improvement.

Identify monitor and manage risk and lead appropriate escalation where required.

Maintain current knowledge of government and service specific guidelines.

Keep up to date with evidence based therapies treatments practices guidelines and frameworks relevant to practice support and supervision.

Keep abreast of current research and identify relevant opportunities for innovation and research.

Support and maintain all accreditation standards relevant to the discipline and services provided.

WHAT YOU NEED TO SUCCEED:

  • Tertiary qualification in a health related discipline directly related to health aged care or a specialist area such as Allied Health or Social Work.

    Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or another relevant professional association.

    Minimum of one year demonstrated experience delivering services in aged care settings and or to older people including people living with dementia within a community setting. This may include experience with RAS ACAT or RAC assessments geriatric evaluation rehabilitation palliative care or community nursing.

    Experience delivering assessment services within culturally and linguistically diverse communities.

    Current Victorian driver licence.
