We are seeking a dedicated and skilled RN Care Manager to join our Clinical Team at our Family Medical Center in Fayetteville AR this role youll be pivotal to delivering exceptional patient-centered care management to individuals with chronic conditions and complex health needs. This dynamic position combines clinical expertise with a passion for promoting wellness and improving patient outcomes. The RN Care Manager works closely with patients families providers and interdisciplinary care teams to design and implement tailored care plans that empower patients to achieve optimal health and self-management.
Through a combination of in-person and telephonic care the RN Care Manager ensures seamless coordination of services reducing inefficiencies and enhancing care quality. Key responsibilities include conducting transitional care outreach within 48 hours of hospital discharge facilitating patient education and supporting self-care initiatives. This role also focuses on achieving targeted outcomes aligned with organizational strategic goals and value-based care measures contributing to the success of the patient-centered medical home model.

Qualifications:
Minimum Qualifications:
- Baccalaureate Degree ADN or Diploma. RN Degree Major: Registered Nurse.
- Valid Arkansas Registered Nursing License
- Minimum of three (3) years RN experience in a clinical environment with current BLS certification.
Preferred Qualifications:
- BSN
- Care management experience in an Ambulatory setting.
- Experience as a participant in continuous quality improvement.
- Knowledge or experience in PCMH is desired.
- Certification in Case Management (e.g. CCM ANCC) or obtain within one (1) year of hire date.

- Strong clinical knowledge and experience in managing chronic conditions.
- Excellent communication and interpersonal skills to engage patients and interdisciplinary teams.
- Proficiency in care coordination and transitional care management.

Nursing Process:
- Triage assess plan implement and evaluate nursing care and patient education via phone or in-clinic using the care management model.
- Develop and manage care plans assess progress address barriers and coordinate care for patients across inpatient and outpatient settings.
- Deliver nursing care aligned with UAMS ANA AR State Board of Nursing and specialty organization standards.
- Evaluate patient progress toward desired outcomes and ensure care meets regulatory and clinical standards.
- Coordinate patient education in collaboration with physician staff.
Education/Evidence-Based Practice (EBP)/Research/Quality Improvement (QI):
- Initiate performance improvement and evidence-based practice/research processes.
