JOB SUMMARY:
The Health Navigator provides health navigation services at the Ruth Ellis Health and Wellness Center. They serve as a liaison link or intermediary between health social services and our youth to facilitate access to services improve the quality-of-service delivery and improve health outcomes. This position also serves as an advocate for patients a resource and a vital part of the Integrated Health team. The Health Navigation educates patients engages and assists them in managing their healthcare needs. This role provides non-clinical paraprofessional duties and is thoroughly familiar with the patients community and the resources available. The Health Navigator collaborates with healthcare staff community-based healthcare social service and other providers.
WHAT YOULL DO:

  1. Gaining continuous knowledge through training personal research and collaboration as systems utilized in this work change and evolve.
  2. Maintaining active participation as part of the interdisciplinary integrated health team with housing case managers medical staff and behavioral health staff.
  3. Assist with patient retention provision of referrals and referral follow-up including to specialty care or external providers. This includes assisting with scheduling appointments and following up with patients who have missed appointments.
  4. Assist patients with insurance navigation including insurance and benefit applications prior authorization insurance appeals updates to ID and legal documents and DWIHN CMH enrollment.
  5. Disseminate health information through one-to-one encounters with patients.
  6. Assess for patient needs and provide referrals to appropriate ancillary services such as housing case management behavioral health medical services etc.
  7. Support patients in navigating mental health care options entering referrals and assisting in enrollment of services.
  8. Work closely with the patient care team to provide information on behalf of patient related needs barriers and facilitators to accessing care thus making it easier to design a tailored care plan for patient.
  9. Provide health education and coaching for patients; for example teach process for refilling prescriptions. Assists in preparation of health education materials.
  10. Participates in community outreach events to provide information regarding Ruth Ellis Center programming and resources available in the community.
  11. Work with an interdisciplinary team to ensure efficiency in clinical care operations.
  12. Maintain a cooperative working relationship with patients and staff by facilitating and enhancing communication and by displaying honesty respect sensitivity and compassion.
  13. Quarterly reporting related to grant funding.
  14. Perform other duties as assigned.
WHAT YOULL NEED:
  • Associate degree or bachelors Degree in a related field or equivalent experience
  • LGBTQ (Lesbian Gay Bisexual Transgender and Questioning) cultural and diversity competency or lived experience with LGBTQ identification and/or homelessness.
  • Strong organizational and communication skills (written and oral).
  • Experience or familiarity with community support services.
  • Demonstrated professional maturity responsibility flexibility and relationship building.
  • Familiarity with the Ruth Ellis Centers principles of work: trauma-informed care harm reduction restorativejustice and positive youth development.
