University Home Care Inc is seeking a compassionate and dedicated Social Worker II (LMSW) to join our team in Livonia MI. We are having open interviews this week from 11am to 4pm Monday to Friday at 11585 Farmington Rd Livonia Mi 48150. Please come see us. This is mostly remote but can be some in person. Lets chat. This role involves providing vital social services to clients ensuring their well-being through assessment planning and coordination of care. The ideal candidate will have strong clinical skills excellent communication abilities and a commitment to improving the lives of those we serve.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify client needs.
- Develop implement and monitor individualized care plans in collaboration with clients and interdisciplinary teams.
- Provide counseling crisis intervention and support services to clients and families.
- Coordinate access to community resources and services to support client goals.
- Maintain accurate and timely documentation in compliance with agency policies and regulatory requirements.
- Advocate for clients rights and needs within the healthcare system and community.
- Participate in team meetings and continuing education to enhance professional skills.
Requirements
- Licensed Master Social Worker (LMSW) credential in the state of Michigan.
- Minimum of 2 years of social work experience preferably in home care or healthcare settings.
- Strong knowledge of community resources and social services systems.
- Excellent interpersonal organizational and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency with electronic medical records and case management software.
- Valid drivers license and reliable transportation for client visits.
