Position: Care Manager
Location: Northern Virginia

We are seeking dedicated individuals who are passionate about making a difference in the lives of older adults. As a Care Manager you will play a crucial role in ensuring that our clients receive the highest quality of support.

Responsibilities include:

Comprehensive Care Management: Oversee and coordinate tailored care plans ensuring that our clients receive appropriate and timely services.
Client Advocacy: Serve as a strong advocate for clients ensuring their needs are met in home medical and care facility settings including while communicating with healthcare providers.
Support and Collaboration: Work closely with team members to address clients needs and contribute your expertise to the team helping to enhance the collective knowledge and skills within the Client.
On-Call Availability: Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients.
Field Work and Documentation: Update family members and other members of the care team and maintain thorough and accurate documentation. Approximately 90% of the role involves field work with 10% dedicated to follow-up and office-based tasks.

Qualifications and requirements:

A masters degree in a related field or a bachelors degree with at least 4 years of relevant experience in gerontology mental health home care or similar.
Available 9am to 6pm MondayFriday and some weekends.
Dependable adaptable and accountable with a strong commitment to providing exceptional care and support to clients.
Must be able to lift and maneuver up to 50 pounds.
Must be comfortable and willing to drive regularly around the Virginia and DC Metro area.

Preferred Qualifications:

Ideal/preferred candidates from a variety of backgrounds including RN OT PT speech pathologist nutritionist social worker.
