POSITION SUMMARYThe Home Care Director provides strategic and operational leadership for Infinites Home Care Department which delivers Home Health Aide (HHA) and Personal Care Assistant (PCA) services across the five boroughs of New York aging adults and Pediatric populations. This leadership role ensures the department achieves financial strength regulatory compliance and exceptional care quality. The Director manages contracts referral relationships and caregiver pipeline development to sustain growth and excellence. A key part of the position is to build systems that demonstrate measurable patient outcomes and faster recoveries proving impact on reducing relapse and hospital readmissions thereby strengthening payer and partner relationships.

Represent the Home Care Department in leadership meetings presenting data-driven updates and improvement and Partnerships:
Negotiate secure and manage contracts with CHHAs and MLTCs that offer competitive reimbursement rates and consistent patient volume.
Ensure all agreements support business goals and profitability standards.
Build and maintain strong referral networks with hospitals physician offices rehabilitation centers case managers and community partners like Care Coordination Organizations (CCOs).
Create and maintain a Partner Plan that includes measurable goals growth budget and initiatives for expanding referral sources.
Use patient outcome data to demonstrate success in improving recovery rates and reducing hospital readmissions.
Continuously identify new partnership opportunities that align with Infinites mission and business development goals.
Build a strategic referral pipeline centered on the aging-adult population to enhance the volume and quality of MLTC eligibility referrals.
Develop a referral pipeline with a focus target market on pediatrics population and individuals with Developmental Disabilities (DD) collaborating with CCOs to enhance the Office for the People with Developmental Disabilities (OPWDD) eligibility referral pipeline.
Develop a strong working association with the home health director to identify patients in need of Home Care Management:
Lead supervise and mentor departmental staff including Intake Staffing/Scheduling Coordinators and Director of Patient Services (DPS).
Provide direction performance feedback and ongoing professional development to enhance team effectiveness.
Ensure efficient coordination of workflows balancing caseloads and staffing to meet service demands.
Foster a positive accountable work environment that encourages innovation collaboration and continuous improvement.
Manage departmental priorities and operational decision-making to ensure timely service delivery and compliance.
Monitor daily census aide utilization and the balance between adults Pediatric and MLTC patient census.
Oversee the DPS and ensure accurate documentation of care plans and compliance with DOH MLTC and CHHA standards.
Maintain strong communication with coordinators caregivers patients and partners to ensure high-quality service delivery.
Implement data-driven management systems that enhance visibility into operations costs and and Growth Oversight:
Develop and manage departmental budgets monitor expenditures and optimize financial performance while maintaining high-quality patient care standards.
Drive financial performance by optimizing reimbursement rates reducing avoidable costs and expanding high-margin service lines without compromising patient care outcomes.
Lead the creation of growth budgets for new business and service opportunities.
Ensure billing accuracy and timely reimbursement from CHHA and MLTC contracts.
Manage inventory and procurement of medical supplies and equipment.
Evaluate reimbursement rates and contracts with partners CHHAs and clients to ensure provider pay rates and profit margins remain sustainable in the long term while complying with legal wage and Pipeline Development:
Develop a strong pipeline of caregivers (HHAs/PCAs) to ensure adequate staffing for current and future growth.
Collaborate with recruitment and PCD to maintain a consistent flow of credentialed and trained caregivers.
Track caregiver retention engagement and training completion to strengthen workforce stability.
Design targeted recruitment initiatives for caregivers that are trained to service pediatric and adult populations.
Oversee orientation performance monitoring and ongoing education for all field and administrative staff.
Foster a positive work environment that encourages teamwork collaboration and a culture of patient centered Compliance and Quality Assurance:
Ensure departmental practices comply with regulatory requirements accreditation standards and healthcare policies.
Lead quality assurance and performance improvement initiatives monitoring clinical outcomes.
Lead preparations for and participate in Department of Health (DOH) and internal inspections and vendor audits.
Manage the Quality Assurance and Performance Improvement (QAPI) program to uphold service excellence.
Implement outcome measurement systems that track clinical outcomes caregiver performance and care quality.
Use outcome metrics to highlight clinical impact that will demonstrate faster recovery reduced relapse and lower hospital readmissions.
Review patient satisfaction results and lead improvement initiatives to increase loyalty and Collaboration:
Collaborate with the Executive Director and other department heads to ensure alignment between strategy and execution and actively participating in organizational decision-making.
Work closely with Finance Department to ensure accurate forecasting budgeting and reporting.
Provide regular performance updates to leadership and maintain open communication across departments.
Represent the department in community and partner meetings.
Collaborate with healthcare providers and interdisciplinary teams to coordinate patient care delivery and optimize clinical Development and Innovation:
Design and launch new initiatives to improve service quality operational efficiency and patient outcomes.
Lead the development of Pediatric and MLTC-focused programs that improve care quality and profitability.
Use data and feedback to identify opportunities for innovation and process improvement.
Promote evidence-based best practices and technology adoption across the department.
Enhance brand and market footprint through active social media management ongoing website updates and the development and dissemination of digital and physical marketing :
Establish primary points of contact for patients families and healthcare providers within the department.
Ensure that assigned staff members manage communication between home health aides and other healthcare professionals.
Address patient and family concerns promptly and Preparedness and Crisis Management:
Develop and implement emergency preparedness plans and protocols to ensure readiness for potential crises or disasters affecting patient care operations.
Lead response efforts during emergencies coordinating nursing staff and resources to maintain continuity of care and ensure patient safety.
Qualifications: Bachelors degree in Healthcare Administration Nursing or a related field (Masters degree preferred).
7 years of experience in a healthcare management preferably in home care strongly preferred.
Proven experience managing CHHA and MLTC contracts with successful reimbursement outcomes.
Strong understanding of Pediatric and MLTC patient care models.
Excellent leadership analytical and negotiation skills.
Ability to manage large field and office teams effectively.
Familiarity with healthcare software EHR systems and HHA Exchange.
