As an RN in LTC your role centers on delivering high-quality person-centered care for residents with chronic conditions post-acute needs and complex medication regimens. Youll assess health status identify risk factors and lead the development and ongoing refinement of individualized care plans in collaboration with physicians therapists social workers and families. Youll administer meds and treatments with precision monitor responses manage wounds and IV therapy as appropriate and implement fall-prevention and safety protocols to uphold dignity and independence.
