Job Summary

Job Title: Sr LTSS Service Care Manager

Location: Source from McAllen Edinburg
Duration: 6 Months (Possible Extensions)
Working Schedule: M-F 8-5 with possibility of OT when warranted

Key Responsibilities:

M-F visits 3 days out of the week with OT possible with manager approval documentation days 2 days out of week traveling to members homes to complete assessments/documentation

Face to Face assessments completed 3 days out of the week with 2 documentation days completing tasks submitting all forms/assessments within turnaround times completing outreaches

Must haves:

Pediatric experience

Nice to haves:

Home Health

Disqualifiers: no experience

License/Certification:

RN License
