We are looking for a passionate and dedicated Community Case Manager to join our Mountain View program. This role provides high-quality person-led support to people living in our supported accommodation helping them enhance their independence community participation and overall quality of life. You will work primarily with individuals who have previously experienced homelessness providing case management coordination and engagement with a wide range of services including NDIS supports and facilitating community-focused activities. This is a role for someone with a genuine passion for supporting the most vulnerable members of our community.
About the role:
- Undertake comprehensive assessments and provide tailored support
- Provide information and assistance across health income employment and social/recreational needs
- Deliver therapeutic support crisis intervention conflict resolution and counselling
- Advocate for clients when engaging with specialist services
- Maintain accurate and up to date case notes ensuring completion of appropriate documentation in accordance with organisational requirements
- Ensure the premises is kept clean and tidy adhering to HSE requirements at all times
The successful candidates will demonstrate the following:
- Diploma or above in community services or other relevant qualification with 3 years relevant experience working with vulnerable people
- Excellent understanding of key issues affecting the homeless and adult Disability community
- Excellent interpersonal and relationship building skills
- Excellent analytical and problem-solving skills
- Excellent time management and organization skills
- Excellent written communication/reporting/email skills
- Current unrestricted Tasmanian Drivers License
