We are looking for a passionate and dedicated Community Case Manager to join our Mountain View program. This role provides high-quality person-led support to people living in our supported accommodation helping them enhance their independence community participation and overall quality of life. You will work primarily with individuals who have previously experienced homelessness providing case management coordination and engagement with a wide range of services including NDIS supports and facilitating community-focused activities. This is a role for someone with a genuine passion for supporting the most vulnerable members of our community.
