DescriptionFUNCTION:
The Community Living Specialist is responsible for supporting people with intellectual and developmental disabilities. This position works with assigned staff to support individuals outcomes and ensures compliance with regulatory internal and external policies. The specialist assists in charting a course that supports each person in realizing their life of personal significance in collaboration with their family and other team members.
RESPONSIBILITIES:
- Coordinates and contributes to the development of plans mandated by regulations ensuring implementation and adherence to each persons person-centered plan and current ISP.
- Revises the ISP to reflect the goals and outcomes chosen by the individual.
- Ensures the accuracy of ISP contents and coordinates the completion of assessments as requested by external partners.
- Respects each persons right to make choices and exercise self-determination supporting their health safety and right to risk.
- Ensures opportunities are provided for each person to explore their interests in the community and form relationships.
- Maintains ongoing communication with families and supervisors to appropriately support each person.
QualificationsThe following combination of experience and education are required:
- Associate Degree from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability.
- Bachelors Degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability.
- Masters Degree from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability.
- Satisfactory Act 33/34 clearances and FBI background check.
- Valid Pennsylvania drivers license and properly insured vehicle (mileage reimbursement provided for travel using a personal vehicle).
- Must obtain First Aid/CPR certification within two months of the start date.
- Successful completion of Medication Administration training provided by Achieva.
