A ProMedica RN Acute Care Navigator plays a key role in overseeing patient care seamless care progression coordination and the appropriate level of care for their caseload. You focus on optimizing length of stay preventing readmissions and compliance with value-based programs.
You will collaborate with interdisciplinary teams to conduct initial assessments and develop discharge plans.
This role also requires patient education through providing patients with information on medications treatments and post-discharge care. Through the patients stay you will monitor their progress review daily health records and escalate any issues to leadership to ensure smooth transitions and patient outcomes.
