| Provide answers to both distributive and clinical questions; consistently monitor the work of lower level staff to ensure adherence to established policies and procedures; resolve problems and issues arising from pharmacy operations and ensure the smooth workflow of services. Work with nurses physicians and other healthcare providers to ensure optimal patient care; work with other members of the medical center to ensure a cooperative work environment; participate in regional or national organizations in pharmacy services; provide information and interdepartmental educational support to all level of healthcare providers (via grand rounds continuing education lectures etc.). Assist in the development and implementation of new policies and procedures and provide input to supervisors and management; lead projects pertaining to the pharmacy area. Assist with training of new and existing personnel on departmental procedures and participate in maintaining quality assurance records. Participate and maintain quality assurance records; participate in continuous quality improvement activities. Serve as patient advocate in matters related to pharmacotherapy; develop staff education materials for medical center staff. Assist with maintaining performance improvement processes in order to improve the medication use process; serve in health system committees and cost savings/quality improvement projects. Provide specialized pharmaceutical care to patients with an emphasis on clinical monitoring and therapeutic interventions to improve patient outcome; assist and provide training to residents and pharmacy students. Contribute to the advancement of RUHS pharmacy services; participatein scholarly research activities and system-wide projects that demonstrate improved patient outcomes. Participate in ongoing medication use evaluation programs by assisting in the establishment of criteria for cost effectiveness/control utilization process indicators and monitoring parameters; conduct evaluations to determine compliance to the established medication use evaluation criteria; summarize results. Contribute to the patient care team by providing medication use expertise to providers nurses and others via multi-modal communication including chart notes SBAR rounding in person debriefs and consultations. |
| OPTION IEducation: Graduation from a School of Pharmacy accredited by the American Council of Pharmaceutical Education with a Doctor of Pharmacy degree (Pharm. D.). Experience: Completion of accredited pharmacy residency Post Graduate Year 1 (PGY-1). Five years of post-residency within current area of specialtyas a pharmacist in a hospital or ambulatory care setting. OPTION II Education: Graduation from a School of Pharmacy accredited by the American Council of Pharmaceutical Education with a Doctor of Pharmacy degree (Pharm. D.). Experience: Two years of specialty residency Post Graduate Year 1 (PGY-1) and Post Graduate Year- 2 (PGY-2). Three years of post-residency within current area of specialty as a pharmacist in a hospital or ambulatory care setting. Option III on job spec is not applicable for this role. License/Certificate: Possession of a valid California Drivers License. Possession of a valid certificate of registration as a Licentiate in Pharmacy issued by the California State Board of Pharmacy and fulfillment of the continuing education requirements. Possession of a specialty board certification by the Board of Pharmacy Specialties (BPS) in a specialty area of existing practice and/or Advanced Practice Pharmacist licensure issued by the California State Board of Pharmacy is preferred. ALL OPTIONS Knowledge of: Drug distribution systems and provision of clinical pharmacy services; system development and implementation of drug distribution and/or clinical systems/services; quality assurance and continuous quality improvement; principles of effective supervision; good customer (physicians nurses and patient) service; professional standards and practices; Federal Board of Pharmacy Joint Commission and DHS laws regulations and guidelines pertaining to patient care and dispensing of legend and non-legend pharmaceuticals and poisons;principles of pharmacy practice drug information education scholarship and leadership. Pre-Employment
