Job Summary
Job Title: Travel LPN Long-Term Care (LTC)
Location: Utica NY 13502
Assignment Type: Travel / Local (Local candidates eligible for local rate)
Duration: 13 Weeks (Extendable)
Shift: Evening Shift 2:30 PM to 10:30 PM
Schedule Requirement: Every other weekend required
Weekly Gross Pay: $1143 $1500
Position Overview
A Long-term care facility is seeking a dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting. The ideal candidate is flexible team-oriented and comfortable floating within the facility.
Facility & Unit Details
Setting: Long-Term Care / Skilled Nursing
Unit Size: 40-bed unit
Floating: Required to all units within the same building
Guaranteed Hours: 37.5 hours per week
Patient Populations:
Medical-Surgical
Orthopedic
Dementia
Memory Care
EMR System: PointClickCare
Responsibilities
Administer medications and treatments per physician orders
Monitor document and report changes in resident condition
Perform wound care and basic nursing procedures within LPN scope
Assist with activities of daily living (ADLs) as needed
Collaborate with RNs CNAs and interdisciplinary team members
Maintain accurate documentation in the electronic medical record
Ensure resident safety dignity and quality of care
Required Qualifications
Active New York State LPN License
BLS Certification (AHA preferred)
Minimum 1 year of Long-Term Care or skilled nursing experience
Ability to float across units
Strong communication and documentation skills
Additional Notes
75-mile radius rule applies for travel candidates
Local candidates are welcome and eligible for local rates
Every other weekend availability is mandatory
