Pay Bonuses & Rewards
- Salary: 27849 per annum generous on-call payments
- Hours: 37.5 hrs per week (9am5pm)
- Contract: Full-time permanent
- Location: Newcastle Office (Gosforth)
- On-call: Rota basis including some weekends (additional pay)

The Role What Youll Be Doing
As a Care Co-ordinator (LIFE) youll be at the centre of how support is planned and delivered for people with Learning Disabilities and Autism. Your role is to make sure support is co-ordinated around each persons needs goals and outcomes matching the right Support Workers skills and approaches so people can build trusting relationships live as independently as possible and work towards the things that matter most to them.
Day to day youll balance people priorities and planning using information data and feedback to shape support solve problems early and keep everyone informed so that each persons support feels consistent person-centred and focused on their aspirations.
In this role you will:
- Coordinate and schedule support across LIFE services making sure each person has the right support in place at the right times to meet their needs goals and outcomes.
- Match Support Workers and individuals based on skills experience communication style and interests to build strong trusting and consistent relationships.
- Plan ahead for changes ensuring support is covered in advance (e.g. holidays appointments changes in need) and responding quickly and safely to unplanned absences or incidents.
- Work with the Trainer and Service Leads to schedule induction training shadowing and debriefs arranging cover or adjusting rota time so learning and reflective practice can happen.
- Maintain accurate records for support plans rota arrangements colleague availability/absences and key contacts/communications on our systems.
- Communicate clearly and compassionately with the people we support their families Support Workers and external professionals (e.g. Social Workers health teams PBS practitioners).
- Respond promptly to concerns or changes in need intervening early helping to resolve issues before they escalate and keeping colleagues informed so we work as one team around each person.
About You
Were looking for someone who is as comfortable with people as they are with planning someone who can balance compassion with clear thinking and good organisation. You will have:
- Experience of coordinating and scheduling (care coordination experience is desirable but not essential if you have strong transferable skills).
- Proven ability to prioritise work stay focused under pressure and manage competing demands
- Strong verbal and written communication skills with the confidence to talk to colleagues families and professionals
- A collaborative approach using information data and feedback to inform discussions and decisions
- A full driving licence and access to a vehicle able to travel as required.
