Job Summary
We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong background in patient care across various settings including acute long-term and community-based care. This role offers an opportunity to work in a dynamic environment where your skills in nursing patient assessment and medical documentation will directly impact patient outcomes. Familiarity with EMR/EHR systems such as Epic Cerner and Athenahealth is highly valued. The LVN will be responsible for providing compassionate comprehensive nursing care in accordance with healthcare standards and regulations including HIPAA compliance. - Conduct thorough patient assessments including vital signs physical examinations and health histories using advanced physiology knowledge.
- Administer medications and injections accurately following prescribed protocols while ensuring proper documentation in medical records.
- Perform IV infusions catheterizations venipuncture and sterile procedures with aseptic technique.
- Manage airway care ventilator support tube feeding and wound suturing as needed.
- Assist with diagnostic evaluations such as cardiac catheterization spinal taps sonography and laboratory specimen collection/processing.
- Provide specialized care for patients with Alzheimers disease dementia traumatic injuries (Level I & II trauma) infectious diseases or requiring hospice or palliative care.
- Collaborate with multidisciplinary teams on discharge planning case management and post-acute care strategies.
- Maintain infection control standards and adhere to NCQA standards for quality assurance.
- Document all patient interactions thoroughly using EMR/EHR systems like Epic or Cerner while ensuring compliance with HIPAA regulations.
- Educate patients and families on health coaching topics including nutrition medication management and chronic disease management such as pain or respiratory issues.
- Valid LVN license in the state of practice.
- Proven experience in diverse clinical settings such as hospitals (including ICU PICU OR) nursing homes assisted living facilities or outpatient clinics.
- Familiarity with ICD-9/ICD-10 coding systems and CPT coding for accurate documentation and billing purposes.
- Proficiency in medical terminology along with physiology and anatomy knowledge relevant to patient assessment and diagnostic evaluation.
- Experience administering injections (including botulinum toxin treatments) catheter care phlebotomy and ventilator management is preferred.
- Ability to work effectively with patients of all agesincluding children (neonatal ventilator experience) and elderly populationsespecially those with developmental disabilities or requiring memory/dementia care.
- Strong understanding of infection control practices and aseptic techniques.
- Competence in using EMR/EHR systems such as Epic Cerner eClinicalWorks or Athenahealth for accurate medical documentation.
- Excellent communication skills coupled with critical thinking abilities to handle urgent situations like acute pain management or emergency medicine scenarios (ER experience).
