Job Description
A reputable healthcare system in southern New Hampshire is seeking a Registered Nurse to join its Diabetes and Endocrinology this role you will assess patient needs coordinate care and support providers in delivering exceptional care. Your clinical judgment and communication skills will help ensure that services are safe efficient and patient-centered.
Key Responsibilities
- Provide nursing care to patients with diabetes and endocrine conditions
- Perform patient assessments vital signs medication reconciliation and clinical documentation
- Support providers during patient visits procedures and diagnostic testing
- Deliver diabetes education including blood glucose monitoring insulin administration continuous glucose monitoring education etc.
- Administer medications and injections per provider orders
- Coordinate care referrals follow-ups and prior authorizations
- Triage patient calls and portal messages related to symptoms medications and glucose management
- Collaborate with physicians advanced practice providers and support staff
Qualifications
- Graduate of an accredited nursing program
- Active RN license
- Prior experience in an outpatient setting is required.

- Schedule: Monday Tuesday Thursday Friday 8am-5pm
- NO WEEKENDS!
