Summary of Duties
To provide a full range of social work services to Neurovascular/Stroke patients and their families. Services include psychosocial assessments counselling discharge planning linkage to community resources and participating as a key member of the patient care team.
Qualifications/Skills
Masters Degree in Social Work
Member in good standing of the Ontario College of SW and SSW
Minimum 2 years of experience working as a MSW in a Hospital preferred
Commitment to Person Centred Care Best Practices
Excellent communication organization skills and time management skills
Strong teambuilding skills and conflict resolution skills
Excellent assessment interviewing and problem solving skills
Knowledge of the health care system and community resources
Superior ability to manage multiple tasks and set priorities in a fast-paced environment
Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education
Commitment to student education
Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care
