Job Opportunity: Nurse Practitioner - Geriatric Medicine

As part of the interdisciplinary PACE team you will support independence dignity and quality of life for participants in our program. Job Responsibilities:
- Perform comprehensive medical assessments
- Develop and implement individualized care plans
- Provide acute illness evaluation and treatment
- Participate actively in discharge planning
- Conduct home visits as clinically indicated
- Coordinate care across the full continuum of services offered by PACE
- Assist in the development and review of clinical policies and procedures
- Participate in provider on-call rotation
Qualifications:
- Board Certified Nurse Practitioner (ANCC or AANP)
- Experience with the frail or elderly population
- Ability to travel within the service area
License and Certification Requirements:
- Active Massachusetts NP license
- DEA Registration
- CDS Certification
- BLS Certification
- Most recent NPDB Self Query Report (within 30 days active)
