Job Summary

Job Opportunity: Nurse Practitioner - Geriatric Medicine

As part of the interdisciplinary PACE team you will support independence dignity and quality of life for participants in our program. This facility excels in delivering services that enable seniors to thrive in their homes while receiving the medical care and support they need with the utmost respect and consideration.

Job Responsibilities:

Perform comprehensive medical assessments
Develop and implement individualized care plans
Provide acute illness evaluation and treatment
Participate actively in discharge planning
Conduct home visits as clinically indicated
Coordinate care across the full continuum of services offered by PACE
Assist in the development and review of clinical policies and procedures
Participate in provider on-call rotation

Qualifications:

Board Certified Nurse Practitioner (ANCC or AANP)
Experience with the frail or elderly population
Ability to travel within the service area

License and Certification Requirements:

Active Massachusetts NP license
DEA Registration
CDS Certification
BLS Certification
Most recent NPDB Self Query Report (within 30 days active)
